CAI2R Collaborators Eric Sigmund, Els Fieremans, Steven Baete, James Babb and colleagues received the Special Scientific Session (SSS) Excellence Award for 2013 at the International Skeletal Society (ISS) meeting in Philadelphia, PA for their presentation on Time-dependent diffusion in skeletal muscle of normal controls and chronic exertional compartment syndrome patients.

Title: Time-dependent diffusion in skeletal muscle of normal controls and chronic exertional compartment syndrome patients.

Purpose: To measure diffusion-time dependence of skeletal muscle DTI in volunteers and chronic exertional compartment syndrome (CECS) patients and interpret it with a random permeable barrier model (RPBM).

Materials and Methods: Seven patients with clinical suspicion of CECS and eight volunteers underwent calf MRI in a Verio 3 T scanner. Stimulated echo DTI was obtained, before and after treadmill exertion, for 4 diffusion times, and analyzed with RPBM to extract free diffusivity D0, fiber diameter a, and permeability κ. Group means of exercise response factors over 7 calf muscle compartments were computed for each parameter and group.

Results: Figure 1 shows radial diffusion and RPBM analysis for a volunteer gastrocnemius medialis. D0 increased by 4.7 % in volunteers and 10.5 % in CECS subjects with exercise. Fiber diameter a increased by 20.8 % in volunteers but decreased slightly (−5.2 %) in patients. Permeability decreased slightly in volunteers (−6.5 %) but increased by 59.3 % in patients. Each parameter’s response factor showed significant (p<0.05) group differences between volunteers and CECS patients.

Conclusion: Fiber dilation may be limited in CECS by intracompartmental pressure, while elevated D0 and permeability κ may suggest edema. These changes, suggested by RPBM analysis of time-dependent diffusion, may help guide CECS diagnosis or treatment.
